Job Summary 

Under the supervision of the Site Clinical Director, and collaboration with the Senior Director of Population Health, the Care Management Supervisor is responsible for providing direct clinical and administrative support to their site care management team.  

 

The Care Management Supervisor will screen, assess, and coordinate linkage for new and existing clients seeking services including primary care at each site. The Care Management Supervisor will partner with the Senior Director of Population Health to analyze data reports and coordinate outreach services to address the needs of clients.  

 

Collaboratively works with the different providers (internal and external) to ensure effective whole person care is provided Ensures goals and objectives specified for the program are achieved within the agreed-upon time framesProvides oversight and direction to team members to ensure client care meets and aligns with the standards of the organization's mission and strategic plan.   

 

Coordinate site operations with other site leadership. Will lead by living the values and philosophy held by Centers and will act as a faithful steward of organization and community resources. 

 

Job Qualifications  

  • Bachelor or Master’s degree in social work with a minimum of three years of related work experience in a non-profit, health-related organization.  
  • LSW or license eligible  

 

Essential Job Duties & Responsibilities 

  • Working closely with the Site Clinical Director on providing leadership and coordination of the care management team at assigned location ensuring all professional, clinical, and ethical standards are adhered to. 
  • Enhances the  care management team by orienting and training new team members as well as developing more experienced team members to be self-reliant achievers 
  • Collaboratively provides individual/group supervision to care management supervisees to ensure the necessary skills are learned and performed  to succeed in the role.  
  • Work closely with the Site Clinical Director to ensure effective performance for direct service programs including timely and accurate documentation and record-keeping by monitoring and analyzing system database and records, and implementing corrective action as necessary. 
  • Ensures proper utilization and timeliness of services and optimizes client care by coordinating services within the organization and with other community support services. 
  • Collaborates with the Site Clinical Director, Counseling Supervisor, and the Senior Director of Population Health on monitoring and report outcomes of the program. 
  • Work collaboratively with the Site Clinical Director on directing the day-to-day clinical operations of the  care management team including assisting staff with scheduling work hours to ensure appropriate coverage for the day to help team members meet the productivity standards outlined. 
  • Co-facilitate team staff meetings and client care meetings. 
  • In collaboration with other team members, will monitor and manage the transition of care notifications and assign staff outreach as appropriate 
  • Collaborates with internal staff and other community providers to develop and provide a continuum of services necessary to promote client success. 
  • Partner with Human Resources staff and the Site Clinical Director to recruit, interview, select, hire, and employ a budgeted number of employees. 
  • Ensures services are carried out under established policies and protocols, ensuring prompt, professional responses to referrals for services, and prompt completion of clinical documentation 
  • Ensures adherence with established service plans, Ohio Department of Mental Health (ODMH) administrative rules, federal, state, and local laws, and professional standards of conduct. 
  • Using the SLII model, empower employees to take responsibility for their jobs and goals. Delegate responsibility and expect accountability and regular feedback. Documents and provides performance feedback to staff regarding the quantity, quality, timeliness, efficiency, and completeness of services performed and efficiency of use of time. 
  • Conduct clinical assessments and provide direct clinical treatment, rehabilitation, and support services to clients, as needed. 
  • Maintain transparent communication. Appropriately communicate organization information through department meetings, one-on-one meetings, appropriate email, and regular interpersonal communication. 
  • Coach, mentor, and develop staff, including overseeing new employee onboarding and providing career development planning and opportunities. 
  • Foster a spirit of teamwork and unity among department members that allows for disagreement over ideas, conflict and expeditious conflict resolution, and the appreciation of diversity as well as cohesiveness, support, and working effectively together to enable each employee and the department to succeed.

  • Assumes supervisory functions in the temporary absence of other supervisors and managers.

Physical Demands/Work Environment 

This job operates in a professional office environment. While performing the duties of this job, the employee is regularly required to speak and hear. The employee is frequently required to sit for extended periods of time, stand, walk, use hands and fingers, and reach with hands and arms. Travel and work outside of normal hours may be required for meetings, presentations, trainings and other events.
