Program:    PRI Transitions
Salary:        $40,000-$46,000

Program Overview:

Project Renewal Transitions provides supportive scatter site housing to 26 chronically street homeless men and women with serious and persistent mental illness/dual diagnoses. The multidisciplinary team includes a Program Director, Occupational Therapist, and two case managers who work with clients to directly deliver and/or facilitate access to a broad range of services geared towards clients’ stabilization, and community integration.

Overall Responsibilities:

Under the general direction of the Program Director, the Case Manager is responsible for providing the required assessment and case management services to address barriers for housing stability for a caseload of 13 clients.

Specific Duties and Responsibilities:

  • Conducts intakes, assessments and psychosocial evaluation as per agency standards for assigned caseload.
  • Conducts a minimum of two home visits per month in apartments scattered through the Bronx.

  • Efficiently utilize digital case management database, AWARDS, to input and update all client information in a timely matter.

  • Provides case management services to assist in the creation of and adherence to Independent Services Plans, identifying short term and term goals and objectives to be achieved.

  • Maintains contact with internal and external service providers on behalf of the client.

  • Identifies service needs and makes referrals as needed, for medical, mental health, psychiatric, substance abuse and other services as needed. Escort as needed.

  • Works closely with Occupational Therapist to enhance clients functional / ADL skills.

  • Participate in clinical team meetings.

  • Performs other duties as assigned by Program Director.

Qualifications:

  • Candidates must have a minimum bachelor’s degree in social work, counseling, psychology, human services, or another related field.
  • Direct experience working with homeless mentally ill adults and providing case management/care coordination.
  • Excellent oral, writing and listening skills.
  • The ability to work well with various deadlines and meet documentation requirements.
  • An ability to interface with clients as well as all levels of staff.
  • Proficiency in Microsoft Office and familiarity with case management software AWARDS.

All newly hired Project Renewal staff must provide verification of being fully vaccinated against Covid-19.
