The primary purpose of this job is to coordinate and assist in the operation of the Medical services for the /patients/participants of Pillar Medical Associates.

____    *Care Manager for practice patients and collaborates with physician and nurse practitioner in role as case manager to minimize ED and hospital transfers.  Requires ability to work face to face and telephonically with patients and families.

 

____    * Prepares patients for examination by physician.  Documents all vital signs and other key indicators as directed, utilizing universal precautions.

 

____    *Initial intake of information for new patient visits.

 

____    *Works with medical assistant and administrative staff to assure new patient records are complete and prepares for review for physician prior to initial new patient visit

 

____    *Calls pharmacy to order and/or renew prescriptions. Requires use of electronic prescribing software.

 

____    *Preliminary review of medication reconciliation whenever patients are discharged from the hospital or rehab center for secondary review by MD/NP.

 

____    *Triages patient telephone calls, assesses needs and directs patients accordingly.

 

____    *Reviews and triages all lab reports, x-rays and other correspondence that correlates with patient visit.

____     *Performs Adult Wellness Visits.

 

____    *Provides direct nursing evaluation and/or other related care and consultation to patients as required.

 

____    *Works collaboratively with Medical Secretary to order clinical supply inventory, requisitions supplies and coordinates restock process.

 

____    *Works collaboratively with medical assistant and other nursing staff when medical or laboratory tests need to be ordered

 

____    *Ensures proper care in the use and maintenance of equipment and supplies; promotes continuous improvement of workplace safety and environmental practices.

 

____    *Assists practice secretary and medical assistant to optimize appointment schedules. ______*Documents all activities in a truthful, timely and accurate manner.

 

______*Assists other practice staff to collect and maintain data regarding patient rosters, diagnoses, and utilization of medical services.

 

____    *Other duties as assigned.

Working Conditions

While performing the duties of the job, the employee is occasionally exposed to toxic or caustic chemicals and infectious diseases.  The noise level in the work environment is usually moderate.  Requires travel (driving) to other senior living communities.

 

Qualifications

Education:

Must have current and valid NYS RN licensure.  BSN preferred but not required.  Minimum 5 years experience as an RN working with geriatric patients.

 

Has existing knowledge of community resources, including but not limited to, senior living communities, differences in levels of care, home health care agencies, social service agencies, hospice, medical testing companies, pharmacies, and durable medical equipment vendors.  Proactively works to maintain this knowledge base as service entities/ vendors evolve.

 

Must have a knowledge and understanding of clinical organizational structure, workflow and operating procedures.  Knowledge of patient evaluation and triage procedures; knowledge of clinical operations and procedures; ability to provide direct evaluation and consultation to patients.
