St. Ann’s Community is looking for a dynamic RN Care Manager for the Pillar Medical Group. The primary focus of this position is to coordinate and assist in the operation of the Medical Services for the patients of Pillar Medical Associates.

Job Description:

  • Care Manager for practice patients and collaborates with physician and nurse practitioner in role as Case manager to minimize ED and hospitals transfers
  • Requires ability to work face to face and telephonically with patients and families
  • Prepares patients for examination by physician.  Documents all vital signs and other key indicators as directed, utilizing universal precautions
  • Initial intake of information for new patient visits
  • Works with administrative staff to assure new patient records are complete and prepares for review for physician prior to initial new patient visit
  • Calls pharmacy to order and/or renew prescriptions. Requires use of electronic prescribing software
  • Preliminary review of medication reconciliation whenever patients are discharged from the hospital or rehab center for secondary review by MD/NP
  • Triages patient telephone calls, assesses needs and directs patients accordingly
  • Reviews and triages all lab reports, x-rays and other correspondence that correlates with patient visits
  • Works collaboratively with Office Specialist to order clinical supply inventory, requisitions supplies and coordinates restock process
  • Works collaboratively with Office Specialist and other nursing staff when medical or laboratory tests need to be ordered
  • Ensures proper care in the use and maintenance of equipment and supplies; promotes continuous improvement of workplace safety and environmental practices
  • Assists Office Specialist to optimize appointment schedules
  • Assists other practice staff to collect and maintain data regarding patient rosters, diagnoses, and utilization of medical service

 

Candidate Requirements:

  • Must have a current and valid NYS RN licensure BSN preferred but not required
  • Minimum 5 years’ experience as an RN working with Geriatric patients
  • Existing knowledge of community resources and clinical organization structure and operations
