Placement Liaison - Wraparound
Behavioral Health Division
Hourly Rate: $19.62 - $28.45
Job Summary: The Placement Liaison Provide direct and indirect support to Children's Community Mental Health Services and Wraparound Milwaukee System of Care Administration Team and Care Coordination agencies around placement of youth in out of home care settings, detention, and inpatient facilities.
Job Responsibilities:
- Coordinate, in partnership with contracted Care Coordination agencies and Placement Providers, resources to meet the emergency and planned placement needs for youth enrolled in the Wraparound Milwaukee System of Care when Wraparound is responsible for securing placement.
- Serve as a liaison between Care Coordination agencies and Child Welfare placement resources.
- Track youth in out of home care settings and conduct regular staffing’s to monitor progress towards permanency and placement stability.
- Monitor youth placed in both Detention and Inpatient facilities. Partner with contracted Care Coordination agencies to facilitate timely and appropriate transitions our of secure placements.
- Review Out of Home Care referrals to ensure completeness and alignment with the Wraparound Philosophy and Process.
- Actively work to create a racially equitable system of care at all levels of program functioning & interactions. Move racial equity forward through equitable & inclusive use of an anti-racist, pro-equity, collaborative approach. Champion/support learning around diversity.
- Perform other duties as assigned to support overall operations of Children's Community Mental Health Services & Wraparound Milwaukee programming.
Minimum Requirements:
- Must be a current resident of the State of Wisconsin
- Current Wisconsin Driver's License and access to a reliable insured vehicle; both maintained throughout employment
- Bachelor’s degree or higher in Human Services, Psychology, Social Work, or in a closely related field. Master’s degree is preferred
- At least one (1) year of related work experience in a Placement related coordination of care is required
- At least an intermediate experience with Microsoft Office software (i.e. Microsoft Word and Excel)
- Basic experience using Zoom/Teams virtual meeting platforms
- Able and willing to travel (approximately 10% of the time) within the community to client’s schools and other settings
Knowledge, Skills and Abilities: Knowledge of WI Children’s Code and youth justice systems, local, state and fed statutes and regulations, guidelines, practices and policies relevant to or specifically applicable to their specific role and operation of youth programs, out of home placement of children, abuse and neglect reporting procedures and requirements, termination of parental rights and children in need of protective services and educational system. Knowledge of human growth and development theories, as well as ACE and trauma related developmental disruption. Knowledge of social work principles and practices; knowledge of case management/assessment techniques, knowledge of mandatory reporter requirements; knowledge of community resources and services available; skill in communication in verbal and written form; skill in interpersonal relationships; skill in organizing office work and cases; ability to communicate case needs to staff; ability to work in stressful situations; ability to work as a member of a mental health team providing crisis services; able to maintain confidentiality; ability to lift, carry, push, and pull up to 20lbs as needed. This position will need to be flexible in working hours with the probability of working outside of the normal 9-5 time frame. On call after hours and responsive while staying within a 40 hour work week is required.
