ADRC Professional – Options Counselor

Hourly Pay Range:  $22.57 - $30.99

The Aging and Disability Resource Center (ADRC) of Milwaukee County Department of Health & Human Services provides FREE information and assistance regarding community resources and services for adults. ADRC staff assist individuals to plan for short-term and long-term needs, access community services and resources, and learn about programs and benefits available. Under the direction of the Aging & Disabilities Services Manager, the ADRC Professional provides a variety of services, including assisting clients with supportive services designed to overcome financial, health, or family problems. This position conducts individual or family assessments to determine services needed, provide agency services, and refer clients to community services. The ADRC Professional works collaboratively with internal and external services to advocate for client needs to assure appropriate services are delivered.

 

Our day to day:

  • Create a positive, courteous environment, and deliver great customer service to customers, colleagues, and Milwaukee County residents. Strive to meet customer satisfaction of ADRC services (i.e., by using motivational interviewing techniques to clarify opportunities to meet customer objectives).
  • Meet with customers including their families, caregivers, and partner agencies to assess the customer’s current situation, goals, and needs. Inform customers of, Long-Term Care program and resources to meet their needs as well as provide support while navigating the complex world of long-term care. Research details of Medicaid eligibility. Educate customers regarding financial eligibility for Long-Term Care programs, the initial application process, cost share, patient liability, and how their income/assets affect eligibility. Ensure costumer needs are being met.
  • Guide customers on how enrollment or disenrollment from Long-Term Care Medicaid programs could impact their specific situation, including financial and personal impact.
  • Conduct functional assessments relating to Activities of Daily Living, such as bathing, dressing, medication management, money management, cognition, memory, and risk, and record findings in the Gateway/FSIA system. Review and research medical diagnoses records as they relate to functional deficits.
  • Achieve Long-Term Care Functional Screen (LTCFS) certification during employment and maintain certification with satisfactory quality reviews. Attend trainings, maintain knowledge of state instructions, and pass the biennial recertification exam.
  • Maintain confidentiality. Document each interaction with customers in the case management system and update records as needed.
  • Participate on internal and external work groups, process improvement projects, county initiatives, and outreach events/activities. Contribute to best practice discussions regarding processes, case consultation, and services.
  • Perform all other duties as assigned.

Is this you?

  • A current resident of the state of Wisconsin
  • Valid Wisconsin Driver's License and reliable insured personal vehicle; both able to be maintained throughout employment.
  • Bachelor’s Degree or higher in Social Work, or a closely related field from an accredited college or university is required
  • At least one (1) year of experience working in a Human Services and/or Social Services field is required. Experience working with adults with disabilities or older adults is preferred.
  • Intermediate knowledge of Microsoft Office Suite (Teams, Word, Excel) required
  • Intermediate experience with CARES, ForwardHealth, FSIA, case management system is preferred
  • Must achieve Long-Term Care Functional Screen certification within one (1) month of employment. Note: Ongoing certification is required by passing biennial examination.
  • Must achieve Options Counseling Standards certification within three (3) months of employment. Note: Ongoing certification is based on State regulations.
