Position Details 

Schedule: Monday-Friday, Flexible, Part Time
Salary: DOE and licensure level, bilingual incentive pay available
Location: Glendale and Mesa locations, Tele-health

  • Provide direct patient care, i.e. medical evaluation and treatment, including medication management.
  • Collaborate with the medical support staff in the coordination of care for all patients.
  • Maintain medical records as set forth by agency policy.
  • Provide medical consultation for patients involved in an inter-disciplinary treatment team, as appropriate to patient care.
  • Be available to take calls and perform on-call responsibilities as assigned by the Medical Director or other site leadership.
  • Maintain communication with other agencies on behalf of A New Leaf patients for coordination of care.
  • Participate in agency meetings, medical staff meetings, and other professional committees as assigned.
  • Maintain an active nursing and DEA License.

Qualifications and Eligibility Requirements

  • Completion of a Nurse Practitioner Program from an accredited school.
  • Must be licensed as a Registered Nurse (RN) and hold a Nurse Practitioner certification from the Arizona State Board of Nursing.
  • Class One Fingerprint Clearance Card
  • Valid Driver’s License with reliable transportation
  • Current Auto Liability Insurance
  • 39 Month Motor Vehicle Record
