The Case Manager functions as a case planner and is responsible for supplying counseling and concrete service interventions to ensure appropriate client care. The case manager is also responsible for assessment and intake, providing therapeutic interventions and counseling.  The case manager is responsible for responding appropriately to crisis situations, and overseeing the range of a client’s services from assessment through to discharge and referrals for aftercare.  Working within the client’s community, develops action plans to effectively identify and utilize existing resources and establish new resources for client care.    The specific requirement for a case manager in the HBCI Program requires that the worker be skilled to manage the counseling demands involved in working with clients who are often chaotic and of highly emotionally charged character.  Many of the clients have been diagnosed with a range of behavioral, emotional, and cognitive disorders such as: Conduct Disorder, Borderline Personality Disorder, Depression, and/or Psychotic Disorders.                                                                                                                                                                                                                                                                                                                                                               TO QUALIFY

  • BA degree and a minimum of two years’ experience in direct service.
  • Some specialized experience with specific client population or specialty (domestic violence, substance abuse, community outreach, etc.) may be required.
  • Valid driver’s license and good driving record
  • Spanish speaking preferred.
