Job Summary:

The Episource Provider effectively diagnoses chronic conditions and provides education and recommendations to promote health and prevent disease progression while maintaining a non-treating/non-prescribing scope of practice. The Episource Provider will conduct a comprehensive assessment which includes a history, a physical examination and clinically indicated or client requested laboratory or diagnostic tests.  All documentation is completed using the Episource proprietary electronic medical record.


Essential Duties & Responsibilities: 

  • Responsible for completing a health assessment of high-risk patients that captures a comprehensive picture of the patient’s disease burden.
  • May be required to travel to remote areas of {local region/state} to perform in-home assessments.  
  • The full time Episource Provider must provide a schedule of availability for health risk assessment (HRA) visits. An individualized schedule will be determined and approved by the clinical supervisor/manager. HRA visits may be performed 7 days per week between 8 AM and 8 PM local time. 
  • Must maintain a HIPAA compliant dedicated space in their home to complete documentation and to conduct telehealth visits as needed. All necessary hardware and software will be provided and only Episource issued hardware may be used.
  • Complete and submit all documentation: adhering to the documentation submission policy.
  • The full-time Episource Provider may be scheduled to perform in-home HRA visits for members which may include travel to remote areas of {local region/state}.  
  • Required to perform the following specialty diagnostic testing procedures including but not limited to diabetic retinal exam (DRE), NC-Stat DPN Check (DPN), bone mineral density testing (DEXA), Spirometry, peripheral vascular evaluation (PADNet). All necessary training will be provided.   
  • Demonstrate accountability for professional practice by participating in educational programs.
  • Respond clearly and in a timely fashion to all communications within the Episource network. 
  • May be assigned other duties. 
  • Must maintain compliance with all company policies and procedures including the Episource Immunization policy.  COVID-19 vaccination is strongly encouraged. 
Additional Responsibilities:

  • May be assigned other duties at the discretion of the Supervisor or Manager of Clinical Services during periods of decreased membership and during any time that available HRA home visits are not filled/scheduled.
  • Additional duties may include and are not limited to clinical QA and chart review.
  • Required to obtain additional state licensure based on Episource needs. 

Qualifications / Requirements:

  • Nurse Practitioner
    • Advanced Practice Registered Nurse (APRN) degree, Master’s degree in Nursing (MSN), or higher (DNP or PHD in Nursing)
  • Physician’s Assistant
    • Bachelor’s Degree or master’s degree for Physician Assistants from a NCCPA program accredited by the Accreditation Review Commission on Education for the Physician Assistant (ARC-PA) or its predecessors.
    • PA-C License obtained by successfully passing the Physician Assistant National Certifying/Re-certifying Examination (PANCE/PANRE).
  • 3 or more years of experience in patient care or clinical work
  • 1 or more years’ experience as an advanced practice provider preferred.
  • Experience performing health risk assessments preferred.
  • Knowledge of risk adjustment methodology, HEDIS/STAR measures preferred.
  • Ability to work in an environment with PHI/PII.
  • Strong attention to detail.
  • Ability to work independently and be self-motivated to meet deadlines and achieve goals.
  • Strong computer skills: Microsoft office preferred.
  • Proficient in the use of Apple / IOS operating system.
  • Strong clinical level of knowledge including clinical assessment skills and chronic disease management.
  • Reliable transportation, current driver’s license and automobile insurance are required.

Working Conditions:

  • Working conditions are normal for an office environment, in the field in member's homes or in the home office setting. May be required to work in a medical office of clinic setting.
  • Appropriate Personal Protective Equipment (PPE) is required and will be provided.
  • Wi-Fi is required. 

Training /Certification Requirements:

  • Current Licensure free of disciplinary actions
  • CRNP license with AANP or AANC Certification
  • PA license with NCCPA Certification
  • Approved Specialties:  FNP, ACNP, ANP, AGNP (not all-inclusive)
  • Company compliance program modules
  • Company safety regulations
