$23.38 - $32.15 hourly Depending on Experience

The Population Health Technician (PHT) in this position is a pre health professional.  The PHT will gain significant clinical experience working with clinicians, staff, and patients at El Rio Health a Federally Qualified Community Health Center (FQHC).  The PHT program aims to improve health outcomes of El Rio patients with chronic medical conditions by directly addressing social determinants of health, as well as lowering patient barriers via outreach, education, referral, and continued follow-up.

The Population Health Technician utilizes motivational interviewing and brief action planning techniques to connect patients with various in-clinic and community resources. The Population Health Technician aides’ clinicians by providing warm handoffs and monitoring a patient’s progress through follow-up calls. Overall, the objective of the Population Health Technician Program is to address the social determinants of health by providing patients with the tools and knowledge needed to improve their quality of life and break down barriers to care.

 

Essential Job Functions:

  • Screens patients for social needs related to social determinants of health. Manage a caseload and evaluate and support patient needs.
  • Coordinates quality improvement (QI) measures based on clinic needs working directly with clinic leadership to set goals on outreach, to develop plans for workflows to improve QI measures.
  • Supports El Rio Health care team in care gap coordination.
  • Finds the best possible service(s) for the patient using clinic resource tools and researching new resources as needed. 
  • Works with individuals and families to create effective action plans to access resources and then follow-up regularly with patients on their action plans until the case is resolved.
  • Tracks, monitors, and documents all patient case work and screening results according to protocol in EPIC.
  • Communicates program highlights, challenges, and outcomes to project leads and coordinator; recommends program improvements.
  • Creates strong and healthy relationships within the health systems, key community-based organizations, and community members.
  •  Assists with researching initiatives, examining local community-focused initiatives, reviewing community assessments, relationship mapping, and local community health worker initiatives.
  • Receives ongoing training through the Population Health Technician Program for responsibilities assigned.  
  • Maintains a clean, safe, and hygienic work environment in compliance with all Policies and Procedures including but not limited to work areas, workstations, examination rooms, hand washing, infection prevention and control etc. for this position.
  • Demonstrates an understanding of and proficiency with the application of all compliance and reporting requirements respective to Joint Commission Certification (JCC) standards.

 Minimum Education and Experience:

  • Bachelor’s Degree with a focus on health care from an accredited college or university.

 

If applicable, equivalent combination of education and experience may be considered, and must be directly related to the functions and responsibilities of the job.

 

Required Licenses, Certifications, and Registrations:

  • Current certification in Basic Life Support (BLS) for health care providers.   
  • Employees in this position are required to have reliable transportation that can meet any operational reassignments of the organization during the workday.  If an employee is driving during work hours, the employee is required to possess a valid driver’s license and must comply with Arizona vehicle insurance requirements.

  • Career interest in Health Sciences.
  • Bilingual (English/Spanish) with the ability to speak, read and write in both languages.
