Position Summary
The Unit Manager / Resident Care Coordinator is responsible to assure the provision of high quality care, compliance, and customer service standards on his/her assigned unit(s) making sure that the medical, physical, social, and emotional needs and general welfare of residents are met. At least 75% time should be spent out on the floor.
Qualifications:
Licensed Practical Nurse with valid current Pennsylvania license
Excellent communication skills (verbal and written)
Supervisory experience a plus
Knowledgeable of Department of Public Welfare personal care home regulations

Essential Duties and Responsibilities

1. QUALITY OF CARE
Reads daily report and follows up to make sure resident care issues are addressed
Reviews the doctor list daily and follows up to make sure resident care issues are addressed
Participates in care planning meetings weekly
Completes initial and annual PCH Assessments using the correct DPW-required form in a timely manner

Makes sure changes and updates to the PCH Assessment are coordinated and documented on the medical evaluation and support plan is knowledgeable of resident care outcome trends and issues on the unit implements strategies for reducing medication errors, resident falls, health care acquired infections, pressure ulcers, call bell response time, hospital transfers, and readmissions to the hospital with same diagnosis within 30 days Copies Medication Administration Records (MARs) at end of month.

2. COMPLIANCE
Assists the Administrator in implementing systems to assure compliance with all applicable local, state and federal regulations and accreditation standards. This includes participation in QA/compliance committees, monitoring, auditing, reporting, mock surveys, dashboards, and DPW inspections.
Completes weekly unit inspections using criteria and format required by the Administrator. Submits the completed unit inspection report including corrective action plan to the Administrator weekly.

3. FINANCIAL
Assists the Administrator in the annual budget process and monitoring revenue and expenditures to make sure services are provided within budget guidelines. Participates in ongoing financial review meetings.

4. HUMAN RESOURCES
Assists the Administrator in all aspects of human resource management including hiring, coaching, counseling, progressive discipline, orienting, serving as staff trainer/facilitator, staff development, recognition, performance appraisals, monitoring care and services. Assures high quality employee onboarding and ongoing training programs.

5. RELATIONSHIP BUILDING/PROBLEM SOLVING
Establishes and builds excellent relationships and communications with residents, families, employees, and community service providers to make sure high quality, compliance, and safety standards are being followed and to obtain feedback for continuous improvement.
Solves problems in a professional, tactful, and timely manner. Proactively communicates “need to know” information to the Administrator. Develops employees to be problem solvers.

6. CODE OF CONDUCT
Commits to high ethical standards and complies with the Code of Conduct, state and federal regulations, accreditation standards, and Concordia policies & procedures. Maintains confidentiality of resident and employee records and information. Has a duty to report compliance concerns, significant legal risk questions, fraud, abuse, suspected or actual misconduct to a supervisor/manager or to the Corporate Compliance Officer.
