We are looking for either a  PT or PTA to administer physical therapy treatments in our inpatient and outpatient clinics located at our Cabot campus to improve quality of life for our community of residents and patients. This position is full time and would operate under daylight hours exclusively. 

Qualifications:

Graduate of a state-approved school of physical therapy or physical therapist assistant program accredited by the American Physical Therapy Association 

Licensed as a Physical Therapist (PT) or Physical Therapist Assistant (PTA) in the Commonwealth of Pennsylvania

Job Responsibilities:

1. Develop and modify individualized, goal-directed treatment plans for residents

2. Oversee all aspects of residents' care plan implementation

3. Conduct safe and effective patient treatment sessions using the prescribed treatment plan, including patient/family education programs.

4. Communicate patient responses to treatment, changes in care plan needs, and progress toward goals.

5. Perform appropriate measurements and assessment techniques within the scope of the Physical Therapy Practice Act

6. Document results of assessment, treatment, follow-up, discharge, and billing notes in an accurate, timely manner in accordance with Standards of Practice, state and federal regulations, and policies/procedures

6. Assist with supervision of physical therapy assistants, students, aides, or volunteers as directed

7. Commit to high ethical standards and complies with the Code of Conduct, state and federal regulations, accreditation standards and Concordia policies and procedures. Maintain confidentiality of patient and employee records and information.
