The mission of the Colorado Coalition for the Homeless is to work collaboratively toward the prevention of homelessness and the creation of lasting solutions for homeless and at-risk families, children, and individuals throughout Colorado. The Coalition advocates for and provides a continuum of housing and a variety of services to improve the health, well-being, and stability of those it serves. 

The Colorado Coalition for the Homeless seeks a Case Manager I to provide comprehensive case management from time of enrollment through program completion and transition into community reintegration for adults who are homeless - including those with disabilities - suffering from drug and alcohol addiction, mental illness and seeking transitional and permanent housing.  Assists Residents with needs related to addiction recovery, primary care, mental health, housing and benefits. The Case Manager is central in understanding every aspect of the Resident’s case in order to continually move each individual through successful program completion and community reintegration. This includes linking residents to local community resources including primary and behavioral health care, employment opportunities and housing supports. Additionally, the Case Manager is responsible for linking residents from Ft. Lyon programs to already existing employment and vocational providers in the community, assisting in job development activities, including education, providing supportive employment services to all project participants, overseeing internal transitional employment positions within CCH, and overseeing other special and/or short-term employment projects. 

Fort Lyon Supportive Residential Community is designed to provide effective and comprehensive services to those who are homeless or at at-risk of homelessness in an effort to end their cycle of homelessness and promote health, wellbeing and active citizenship. These supports include direct access to housing with enhanced services including education, vocation and job training opportunities, linkage to co-occurring behavioral health and primary health care, acquisition of income, benefits and entitlements.  The program staff works as a team to ensure a continuity of care. This position is located at 30999 CR15, Las Animas, CO 81054.

Essential Job Functions

  1. Participates in intake, orientation and engagement of new Residents.
  2. Provides ongoing engagement and assessment of Residents in order to develop and update person-centered, strengths-based Goals and Objectives (GO) plans.
  3. Facilitates access and follow-up with community based primary and behavioral health care providers.
  4. Meets with every Resident on assigned caseload a minimum of 2x/month to assess progress, assist with daily and weekly schedule, conduct room check, review GO plan and schedule staffing as necessary.
  5. Evaluates residents’ needs through regular contact, including crisis intervention as necessary.
  6. Manages each Resident on assigned caseload from intake to community transition.
  7. Provides regular group meetings with entire caseload to help build community and share resources.
  8. Assists with managing the Residential Community milieu through regular, informal contact with Residents, both on and out of assigned caseload.
  9. Provides backup case management coverage as needed.
  10. Provides transportation for Residents as necessary.
  11. Acquires goods and services for Residents based on determined need (e.g., housing, personal identification information, benefits acquisition, health insurance, food, medication, treatment etc.).
  12. Participates in team and supervisory meetings as assigned.
  13. Performs additional duties as assigned. 

Qualifications Summary

  1. Bachelor’s degree in social work, counseling or similar field preferred. Relevant experience may substitute for education/training.
  2. Experience working with disenfranchised persons a plus.
