BREIF OUTLINE OF DUTIES Review Health plans of each resident. Develop and manage the self-administration of medication program for residents. Insure proper documentation is maintained on the administration of medication, verbal physician’s orders and individual self medication plans. Act as a liaison between physician and program staff. Provide training to residents and staff in hygiene, medication side effects, first aid, and nutrition. Participate in treatment team meetings. QUALIFICATIONS AND EDUCATION REQUIREMENTS Valid Pennsylvania license as a LGPN plus some experience working with MH population, psychotropic medications and the chemically dependent. Specific duties include but are not limited to: Escort clients to all meals. Run 12 Step Education Groups. Take intakes and verify clients’ insurance status. Monitor clients when they are not in treatment groups. Document client behaviors throughout the shift. Deal with crisis emergency situations as outlined in Rivers Bend Policies and Procedures Manual. Transport Clients to appointments. Ensure that information is communicated to the clinical team or program leadership. Accompany clients to various medical and social service appointments. All other duties as stated and/or assigned by the Facility Director. Qualifications and Education Requirements High School Diploma is required, plus at least six (6) months experience working in the Mental Health/Drug and Alcohol field. Must have a valid Pennsylvania Driver’s License and drive clients to appointments with Agency Vehicle. Brief description of job Completes Personal Data Summary on assigned residents. Interacts with residents on a daily basis. This involves weekly meetings to teach residents life skills, such as shopping, clean-up of room, grooming and personal care. Assists residents in learning to use community resources, public transportation when needed and accompanies them to appointments when necessary. Is responsible for timely record keeping on assigned clients. Facilitates interaction and socialization among the residents. Provides crisis intervention when necessary. Functions as a client advocate. Is responsible for the security of the residents and for the care of the building and property during assigned shifts. Attends departmental staff meetings and in-service training sessions. Performs other duties as assigned by Supervisor. Qualifications and Education Requirements Prefer a Bachelor’s Degree in one of the social sciences and one year of mental health experience. Or One to two years of college, and/or experience providing services to the mentally disabled. Or High School Diploma (or equivalency) Plus twelve (12) semester credit hours from an approved educational program and two years direct care experience. And Excellent communication skills, a high degree of maturity and willingness to work with the chronically mentally ill and a valid PA. Driver’s License.
